Chrome and Safari Versions:
Go to Settings, then Print. Set to Portrait then clear Headers and Footers. Pull the margins to 0 inch on all four sides.
At the time of this writing Safari for Windows lets you set the margins, but not remove the header and footer. Safari for Windows seems to be receiving no further development so this deficiency may never be improved.
IE8+ and Firefox Later Versions:
Go to File, then Page Setup, check the Print Background Colors and Images box, clear Enable Shrink to Fit for IE.
Under the section labeled Headers and Footers set the top and bottom margins to 0 and the left and right side margins to 0.
Set all the Headers and Footers to -Empty- or Blank.
IE7 and below:
Go to File, then Page Setup find the section labeled Headers and Footers. So you can return it to its original state copy the characters listed under the Header and Footer fields.
Then, delete all the characters in the Header and Footer fields. Set the top and bottom margins to 0 and the left and right side margins to 0.
Firefox Earlier Versions:
In Firefox go to File, then Page Setup, click the Headers and Footers tab.
Set all the drop down boxes to --blank-- then set the top and bottom margins to 0 and the left and right side margins to 0.
Opera Versions:
In Opera go to File, then Print Options, clear the Print Headers and Footers box.
Set the drop down box Scale Print to 100%. Next, you want to set the top and bottom margins to 0 and the left and right side margins to 0.
